When you are hurting, time is of the essence. That is why we offer same day service, online health and medical history forms, convenient office hours and emergency appointments. We are pleased to offer Care Credit to our patients. Care Credit is a convenient, no initial payment, low monthly payment plan for dental treatments. Offering Care Credit allows us to make the treatment you need affordable. For more information or to apply online log on to Care Credit.
As a courtesy service to you, we are happy to assist you in submitting your claim to your dental insurance carrier. We ask that you provide us with all of your insurance information at the time you schedule your appointment. Be advised insurance companies routinely indicate that coverage verification does not guarantee payment. At the time of service, you will be responsible for the full fee.
Patients with Excellus/BCBS will be responsible for the patient’s portion as determined by BCBS at the time of treatment. If your benefits are exhausted, maxed out, placed under review and denied, YOU ARE REPONSIBLE FOR OUR PRIVATE FEE SCHEDULE NOT THE EXCELLUS RATE.
The quality of your care is very important to us. As a result, we do not “double book” our patients so that we can focus only on you. Once an appointment has been made, this time has been reserved exclusively for you with the appropriate staff to serve your dental needs. We would appreciate 24-48 hours notice prior to canceling.
